[agentic azd] Error handling tool and error middleware

• Create azd error troubleshooting tool on top of mcp PR
• Use agent mode in VSCode to run azd provision command and fix the error using azd error troubleshooting tool (Must be specified in prompt)
• Improve azd mcp server description to increase the possibility of azd error troubleshooting tool when error occurs for azd commands
• Implement error middleware so that cli has vscode agent workflow experience where it passes error to agent and fix it until command runs successfully

I think we might want to add a couple more things like:

• Allow passing optional additional context to the error middleware
  • This could be information about the action the user was attempting
  • This could be details about the app's resources/state/etc.
  • This could include our own instructions on how to fix this class of error
• Sniff for common errors with subpar messages that we know cause a lot of customer confusion
• Provide additional context for the top 3 common errors (TBD) we've seen from telemetry/issues
  • If we have an agent, we can give that context to the error middleware
  • If the user doesn't have an agent enabled, then we can still print out the extra context for them to consider themselves
• Investigate adding a deep link to logs for Azure specific errors

This should both help the AI give better responses and also benefit existing customers who might not opt-in to agentic azd right away.
